Maybe the babysitter is on the way over. Or your son is heading off to soccer practice. Or your daughter is starting third grade. It is important to make sure people have access to information about your child's epilepsy. That's why we've created easy-to-use Care Cards. Simply fill out a card with important information, like contact numbers and signs of a seizure. Have your child carry one and share with people who may need to know what to do if your child has a seizure. It is easy, valuable and helps people understand epilepsy better.
Identification & Emergency Card
Patients or their caregivers can fill out this wallet-sized card with their name, address, treatment details and whom to contact in case of a seizure.

Patients or their caregivers can give this card to teachers, coaches and anyone else they interact with who may be with them at the time of a seizure. It includes everything from information about their epilepsy to doctor and parent contacts.
