This downloadable Interactive Seizure Diary helps you track your seizure activity and medication routine. You can also record doctor appointments and notes about how you are feeling. Before a doctor appointment, print the information and share it with your physician.

This application is compatible on a PC with either Windows 2000 or Windows XP. If you don't have a compatible computer or if you would prefer a printed version rather than a application on your computer, please use the Seizure Diary PDF provided below.
PC Download:
Right-click on the link below and choose "Save Target As..." to save the application to your desktop. Once the Interactive Seizure Diary has been downloaded, double click on the desktop icon and follow the installation instructions.
This Seizure Diary can also be used to track occurrences of seizures. Download it, print it out and then record your seizure activity. This diary comes in Adobe Portable Document Format (PDF) and requires Adobe Acrobat Reader. To get Adobe Acrobat Reader, click here.
